You've probably had your blood pressure taken dozens of times — at the doctor's office, at the pharmacy kiosk, maybe even at home. The nurse wraps a cuff around your arm, a machine inflates it uncomfortably tight, then announces two numbers separated by a slash. "One-twenty over eighty," she says, and moves on.
But what does that actually mean? Most people leave with no real understanding of the measurement, which is a shame, because those two numbers say a surprising amount about what's happening inside your cardiovascular system right now — and what might happen to it in the future.
The two numbers: systolic and diastolic
Blood pressure is measured in millimetres of mercury (mmHg) — an old unit from when mercury manometers were used to measure pressure. The cuff works by cutting off blood flow in your arm artery, then slowly releasing the pressure while listening (or sensing electronically) for blood to start flowing again.
The top number — systolic pressure — is the force your blood exerts against artery walls when your heart contracts and pumps. It's the maximum pressure in the system. The bottom number — diastolic pressure — is the pressure when your heart relaxes and refills between beats. It's the minimum, the baseline your arteries are always under, even at rest.
So 120/80 means: 120 mmHg of pressure during a heartbeat, 80 mmHg between beats. Both numbers matter, and different patterns of elevation can point to different types of cardiovascular risk.
What the ranges actually mean
The American Heart Association (AHA) defines the following categories:
- Normal: Below 120/80 — the target for most adults under 60
- Elevated: 120–129 systolic with less than 80 diastolic — not yet hypertension, but a warning sign
- Stage 1 hypertension: 130–139/80–89 — lifestyle changes recommended; medication may be considered
- Stage 2 hypertension: 140+/90+ — medication is typically recommended alongside lifestyle changes

- Alcohol: More than moderate alcohol consumption (roughly 1–2 drinks per day) is consistently associated with higher blood pressure.
- Smoking: Nicotine causes immediate, short-term spikes in blood pressure and long-term arterial damage.
- Stress: Chronic psychological stress activates the sympathetic nervous system, keeping circulating adrenaline and cortisol elevated, which raises heart rate and vascular resistance.
- Genetics and age: Blood pressure tends to rise with age as arteries stiffen. Genetic factors account for an estimated 30–50% of individual blood pressure variation.
Understanding isolated systolic hypertension
One pattern worth understanding separately: isolated systolic hypertension, where only the top number is high (say, 160/75). This becomes more common with age as the large arteries lose elasticity. Stiff arteries rebound less during the diastolic phase, keeping that number low or normal, while the systolic spike from each heartbeat is amplified.
For decades, isolated systolic hypertension was considered a normal consequence of aging and left untreated. We now know it significantly increases stroke and heart disease risk — treating it reduces major cardiovascular events even in people over 80.
How to get an accurate reading
One elevated reading doesn't mean you have hypertension. Blood pressure is highly sensitive to circumstance, and a single clinic measurement can easily be misleading. For accurate results:

- Avoid caffeine, exercise, and smoking for 30 minutes prior
- Use the correct cuff size — a cuff too small can artificially inflate the reading by 10–40 mmHg
- Take two or three readings a minute apart and average them
- Measure at the same time of day for comparisons — blood pressure is typically lowest in the morning and highest in the afternoon

What lifestyle changes actually do
The evidence for non-drug interventions is genuinely strong. According to the AHA's own meta-analyses, each of the following can reduce systolic pressure by roughly the amounts listed:
- Weight loss (per 10 kg lost): −5 to −20 mmHg
- DASH diet (vegetables, fruit, low-fat dairy, limited saturated fat): −8 to −14 mmHg
- Regular aerobic exercise (150 min/week): −4 to −9 mmHg
- Sodium reduction (to under 2,300 mg/day): −2 to −8 mmHg
- Limiting alcohol: −2 to −4 mmHg
These are meaningful effects — comparable to what many first-line medications achieve. The catch is that lifestyle changes require consistency, and most people don't sustain them. Blood pressure doesn't exist in a vacuum. A reading of 135/85 means something different in a 30-year-old with no other risk factors than in a 55-year-old who smokes, has elevated cholesterol, and a family history of heart disease.
